Community Health Partners, as part of Community Care of North Carolina (CCNC), has received funding from the Federal Children’s Health Insurance Program Reauthorization Act (CHIPRA) to participate in a demonstration grant. The grant goal seeks improved access to care and to improve the quality of care for children by evaluating a core set of measures under Medicaid or the Children’s Health Insurance Program service delivery.

Community Health Partners employs a part time quality improvement position that provides leadership for the grant and partners with our Primary Care Physician (PCP) network pediatric practices. They together collaborate on quality improvement plans and projects within those practices by collecting and reporting on practice level data. There are 24 core quality measure indicators that relate to the grant; and, our practices adherence to meaningful use criteria as recommended by Agency for Healthcare Research and Quality (AHRQ).

Community Health Partners and our Pediatric Physicians network are implementing and evaluating the use of these core quality indicators as we continue to identify, treat and provide coordination of care for children in the network. This grant project allows us to strengthen our medical home model for the children with a physician led community based network in our community.
